Applying for Ganister Station
Ganister Station provides community participation services to adults, 18 and older, that have been diagnosed with and intellectual disability.
Referral Process Disclaimer
Referral processes may vary by county, please check with your supports coordinator for more detail on your counties programming referral process, as well as your counties requirements for starting programming.
Generally, a referral is made by a county support coordinator by contacting the program specialist at the building the individual wishes to attend. The program specialist will be able to provide information regarding the program, and what steps need to be taken to begin attendance.
Referral Process Steps
Prior to starting programming, all individual will need to complete the following steps, referral times vary based on how quickly these steps are completed
Step One
A tour of the facility will be held, this will give all parties involved an opportunity to meet in a low stress environment. This gives all involved an opportunity to ask questions about supports and services offered at Ganister Station, as well as the ability to meet the individual that is seeking services.
Step Two
A copy of the individuals ISP (individual support plan) must be provided for review prior to admission. This gives us the opportunity to review their needs to be sure we can properly support them.
Step Three
We ask that the individual complete three visits to the facility, 2 with supervision, either from a family, or support staff (supports coordinator, residential/home and community staff, behavior specialist etc.) that knows them well, and 1 solo visit to the facility with someone available to call in case of emergencies. These visits can vary in length of time, and serve to help us get to know the individual.
Step Four
A team meeting is then scheduled to discuss days of attendance, times, and staffing ratios, as well as other special considerations such as dietary restrictions, medications, etc.
Step Five
A current physical, medical history, current list of medications, and immunization records, as well as releases of information must be provided to the program prior to admission. This information can be found in the Ganister Station welcome packet, which will be provided during the facility tour.
