Applying for Ganister Station’s Adult Day Care Program
Our adult day care program admissions are generally handled by the local Mental Health/Intellectual Disabilities office of the county in which you reside. Please reach out to your local supports coordination organization to see if you qualify for services. You can also call one of our locations listed on the contact page of our website or click on Contact Us below and fill in the box with your information and select Day Program Services in the subject line. One of our administrative staff will reach out to you to provide information to you regarding admissions to the day program. The staff person will be glad to talk to you about the services the day program provides and what steps are needed to see if we have a suitable vacancy and availability at one of our adult day programs.
Persons seeking services need to complete an application that includes medical history, physical, Individual Support Plan, and any additional information which will help us in meeting personal needs.
It is important that we understand the personal, physical, and emotional needs of the person in order to provide a safe and comfortable environment.
Attending a day program will enable you or your loved one to meet new people and participate in a wide variety of daily activities to gain opportunities to grow, change, and develop greater independence while experiencing an everyday life.
