Program Specialist

Qualifications

  • Master’s degree or above from an accredited college or university and 1 year work experience working directly with persons with intellectual disabilities
  • Bachelor’s degree from an accredited college or university and 2 years work experience working directly with persons with intellectual disabilities
  • Associate’s degree or 60 credit hours from an accredited college or university and 4 years work experience working directly with persons with intellectual disabilities
  • Must complete eligibility requirements, physical examination, and a criminal record check (Act 33 clearances)

Responsibilities

  • Responsible for a maximum caseload of 30 individuals
  • Coordination and/or completion of assessments
  • Providing the assessment for the development of the ISP, ISP Annual Update, and all ISP revisions as required under 2380.181(f)
  • Participating in the development of the ISP, ISP Annual Updates, and all ISP revisions
  • Attending the ISP, ISP Annual Update, and all ISP revisions
  • Fulfilling the role as Plan Lead as applicable under 2380.182,  2380.186 (f) and (g)
  • Reviewing the ISP, ISP Annual Update, and all ISP revisions
  • Reporting content discrepancies to Support Coordinator as applicable and Plan Team members
  • Implementing the ISP as written
  • Supervising, monitoring, and evaluating services
  • Reviewing, signing, and dating the monthly documentation of an individual’s participation and progress toward outcomes
  • Reporting a change related to the individual’s needs to the supports coordinator as applicable, and Plan Team members
  • Reviewing the ISP with the individual as required under 2380.186 (relating to the ISP Review and ISP Revision)
  • Providing documentation of the plan review to the Supports Coordinator as applicable, and Plan Team members as required under 2380.186(d)
  • Informing Plan Team members of the option to decline the ISP review documentation as required under 2380.186(e)
  • Recommending a revision to a service or outcome in the ISP as required under 2380.186(c)(4)
  • Coordinating the services provided to an individual
  • Coordinating the training of direct service workers in the content of  Health and Safety needs relevant to each individual
  • Developing and implementing provider services as required under 2380.188 (relating to Provider Services)
  • Transport to and from Day Program
  • Be available every day to work floor to support staff during morning activities, meal time, and special activities
  • Meet all Ganister Station employment policies
  • Be committed to supporting the individual in achieving the life they want
  • Other duties as assigned
